Social Anxiety Disorder Therapy

Treatment Specialties

It's more than shyness. Social situations trigger intense fear of judgment, embarrassment, or being watched — to the point where you avoid things you'd otherwise want to do. Social Anxiety Disorder involves an intense, persistent fear of social or performance situations where you might be scrutinized or negatively judged — a work presentation, a party, even ordering food at a restaurant.

Physically, it can show up as a racing heart, sweating, nausea, or a mind that goes completely blank. Mentally, it often comes with a running commentary of self-criticism before, during, and after social interactions — replaying conversations for hours afterward, certain you said the wrong thing.

Over time, avoidance becomes the coping strategy: skipping events, staying quiet in meetings, turning down invitations. It works in the short term and quietly shrinks your life in the long term.

Why avoidance makes it worse, not betterThis is one of the most important things to understand about social anxiety: avoidance provides immediate relief, which is exactly why it becomes such a strong habit.

But every time you avoid a feared situation, you confirm to your brain that the situation was dangerous enough to avoid — which makes the fear stronger the next time, not weaker. Breaking this cycle is central to effective treatment, and it's not about "just doing it" through sheer willpower. It's about doing it in a structured, supported way that actually works.

Treating Social Anxiety

At Prime Therapy Services, our primary approach is Cognitive Behavioral Therapy, specifically using cognitive restructuring and gradual exposure. Cognitive restructuring helps you identify and examine the automatic, often distorted thoughts driving the fear — "everyone is judging me," "I'll humiliate myself" — and test whether they hold up against evidence.

Gradual exposure involves building a step-by-step hierarchy of feared social situations, starting with something manageable and working up to more challenging ones, always at a pace you help set with your therapist. This isn't about being thrown into your worst fear on day one. It's methodical, collaborative, and designed so that each step builds real confidence for the next, rather than overwhelming you into further avoidance.

What treatment actually involves

Your Prime Therapy Services clinician will work with you to map out the specific situations that trigger anxiety and rank them by difficulty, then help you practice facing them — sometimes through role-play in session first, then through real-world "behavioral experiments" between sessions. You'll track predictions versus outcomes: what you feared would happen, and what actually happened. Over time, this evidence becomes more persuasive than the fear itself.  NIH statistics on social anxiety disorder help us to inform our treatment approach.

Social anxiety treatment is available in-person at our private NJ office and via Telehealth throughout New Jersey. Some clients specifically value in-person sessions because practicing social presence in a real room with another person is itself a form of exposure; others prefer the lower-stakes entry point Telehealth offers, especially early in treatment. Either path is fully supported.

Where sessions happen

Your first session focuses on understanding exactly which social situations feel hardest, what avoidance currently looks like in your life, and what you're hoping treatment makes possible again — whether that's speaking up at work, dating, or simply feeling less exhausted after social events.

What the first session looks like

Social anxiety often convinces people that reaching out for help is itself a social risk — one more way to be judged. It isn't. This is one of the most researched, most treatable anxiety conditions there is, and asking for support is the opposite of the weakness social anxiety tries to convince you it is.

You don't have to "get over it" alone

Frequently Asked Questions

Is social anxiety the same thing as being introverted?

No. Introversion is a personality trait involving a preference for lower-stimulation environments. Social Anxiety Disorder involves significant fear and distress around social situations that can interfere with work, relationships, and daily functioning.

Will treatment force me into uncomfortable situations right away?

No. Exposure is built gradually and collaboratively, starting with manageable situations and working up at a pace you help set with your Prime Therapy Services clinician.

How long does treatment usually take?

Many clients see meaningful improvement within 12 to 20 sessions, though this varies based on severity and how long avoidance patterns have been in place.

Can social anxiety be treated alongside other conditions like depression?

Yes. Social anxiety frequently co-occurs with depression or generalized anxiety, and treatment can address multiple concerns together.

Is Telehealth effective for treating social anxiety?

Yes. Many clients begin with Telehealth, which can feel lower-stakes initially, and some transition to in-person sessions as part of their exposure practice.
